e-mail to/from people unknown

I've been receiving e-mail from unknown people and it goes directly to my
junk mail to be deleted. But I've just discovered, by requesting delivery
verification on an e-mail out, that my system has started to send to people I
don't know. How can I stop this? I've run McAfee virus scan, I have
personal firewall and privacy service with McAfee also. Everything comes
back clear on the virus scan.
 
who is it sending to and what is it sending to them? Is the server doing it
(you can't stop that) or is outlook doing it?
