double entry from forms

I have forms on my frontpage administered website. When
the results of the form updates a file or sends an email
it is duplicated. Can any one tell me how to stop this
so only one email is sent and only one file entry is made?

Hi Dave,

Does this happen every time..or only occasionally? The reason I ask is
sometimes people become a little impatient and hit submit more than once
(this can cause multiple entries) Usually a little note saying please be
patient and only submit once (or something similar) will stop some of that.
