Disappearing calendar items

Exchange 2003/Outlook XP and 2003

I have users that will have appointments on their calendar but their
free/busy will look open even though there's a meeting scheduled for that
time (and the meeting's not marked as private). By starting Outlook with
/cleanfreebusy usually fixes the issue (temporarily) until a week or so
later. Anyone have any ideas? It's usually conference rooms or executives
that have a lot of recurring meetings.

Figured you may ask, BlackBerry Enterprise Server 4.0 SP1 and 4.0 on the
Blackberry devices.

We are experiencing the same problems with the our Executive Assistants.

What is the interaction with the Blackberries?

Is there a technical solution? or training ?
