Custom Icons

Using XP

I should like to make some easily recognised custom icons for a visually
handicapped friend

Some of the standard icons are useful but it would be nicer to be able to
make some which represent the contents of the folders (and possibly some
documents)

1 Do you know of a reliable freeware icon maker?

2 Where are the MS icons stored? Can I simply put newly made icons there?

3 Have you any other advice than "don't go there!"?
