Custom Form size

I have an OKI Pacemark 4410 forms printer that I use to
print continuous form 1-up labels with. In Windows 98 I
was able to create a user defined size of 5" x 1.5". In
XP I have created a custom form with this size. How
can I get this new form to appear in the available forms
pick list for my printer? It is a network printer. Thanks
 
I have the same problem with and Epson AP5500. I created a form in Server properties of .5" x 3" for slide labels. This doesn't show up in paper sizes though when the printer is selected. Neither does custom in the list.
