Custom Form- Display Name

I have recently created my first custom contact form in Outlook to be used as
a database for a foundation where I work. It is all working really well
*except* that the 'display name' (the blue banner at the top of each contact)
is acting weird. Every form is coming up with a heading of 'Richard L.
Jones' and this doesn't change even when I enter new info in the
company/contact fields on the form. I have tried going to the folder and
selecting 'Properties' to change this, but no matter what I put in it still
has "Richard L. Jones" at the top of each contact. Help! Thanks...

By "blue banner," do you mean the title bar of the individual item window? Or the information in a folder view?

With the form open in design mode, look on the All Fields page under All Contact Fields to see if you find any clues there.

yes, i did mean the title bar of the individual item - and
thank you SO much, you have saved me from pulling my hair out. that fixed
the problem, you are wonderful.
