Custom Buttons Disappear

I'm programming in VBA in Office Outlook 2003. I'm Having issues with my
custom button disappearing after closing the program. Is there a way to embed
the button in to outlook?

Macros are enabled at start up.
 
Please show the code for how you create the button.
