creating totally new Contact Cards

I would like to create and keep new contact cards for a project I am working
on. I need name, site, date of event, and several other odd fields.
I did design a form, and saved it, but I cannot figure out how to use it the
same way I do normally; that is, I want to be able to use the custom card,
fill it out, click "save and new" and have the same card come up time and
again.
I must be missing something....
Also, once I have (hopefully) gotten that down pat, will I be able to create
a Distribution List using those same custom cards?
I sure hope someone can help me! And thank you in advance!
 
Right click on the Contacts folder, select properties, and use the dropdown
to select the default form used "when posting to this folder."
