Creating Outlook templates

I have created a form in MSWord and would like to turn it
into a template that I can add to my Outlook forms. How
would I go about this task?

Word forms and Outlook forms are two different kinds of forms. You can't
easily convert one to another. You'd need to design an Outlook form from
scratch or, if the Word form is a document with form fields, send it as an
attachment.
