Creating blank lines in a form with labels underneath lines

I'm trying to create a form with blank lines and labels under the lines and can't seem to get it to work out.

You have to put the text on two separate lines. Type the underlines in the
first line and the text on the next line. You'll probably want to use a
smaller font and close up the spacing considerably.
