Creating a template in Outlook 2003

Can someone provide instructions on how to create a template so I can email
several people and have it personalized to the individual recipients. The
HELP instructions on designing a form are not very clear. I have an excel
database with fields I want to link to the email.
 
This is rather a large task, which would involve at a minimum:

1) Code to create duplicate messages, each to a single recipient.

2) Database connections

Custom forms info starts at http://www.outlookcode.com/d/forms.htm, but what you describe are intermediate to advanced tasks.
