What do I need to do in order to make this form at

JavaScript or a server-side technology like ASP or
ASP.NET. There are also some third-party programs that
can do this.

....but, if you intend to use Paypal as your form suggests, you don't need to do what you are asking...Paypal will do all that for ya.
