creating a form that I can type in

In an old version of Word (97 maybe?) I was able to create a document
template with form fields that I could then type in. (Used for a student
report - so all the headings were there, etc, and I could type in name, date,
etc. ) I saved these as templates. When I try to do that now, in Word 2003,
I can't type in the form fields without all the following text moving, which
defeats the purpose. Am I saving it incorrectly? If not, what could I be
doing wrong? Thanks for any help.
