Create forms in Outlook 2003

I have created a form in Outlook 2003. I've published the form but when using the form, the text box fields and check box field are not showing on the receiving end. Any help is much appreciated.
 
So does anyone have an answer for this? The problem I have is, perhaps during the design steps, when receiving the form the custome build fields and textboxes are not showing. Any idea why?
 
It seemed this forum is pretty useless so I'll take my question to some other place.
