Create Date Field in Custom Form

I am trying to create a custom form that has a date field. It will be a
Time-Off Request form. I want the user to be able to click the down arrow,
view a calendar, click on the date, and have it populate the field with the
proper date and format. The form that I started with is a mail message form
since it must be emailed to a manager for approval. I want the field to work
and look just like the creating a calendar item when selecting the start and
end dates for an appointment.
 
Outlook itself does not include any date picker control (which, by the way, is one reason why the first page of the appointment, journal, and task forms is not editable). See http://www.outlookcode.com/d/datepick.htm for controls you can add.
