Create a transparent form

I would like to create an email that includes a rental application form in which the recipient could fill out and return. I would like the text of my form to be unchangeable - just let the recipient add text over the indelible form. Any help is appreciated. I know how to create .gif's or jpg's from a word file - but when I insert them into the Outlook e-mail message body, the user erases the object once they are start inputting text.

Why not just create the rental agreement in Word using Fields and locking
the text, just allowing the person to fill in the fields for things like
name, address, social security number, etc.

Then send this form to people for completing a rental application.
