Create 5x8 contact card with fields to import from Access.

I would like to know how to create a form using word 2000 to import data from
Access or Excel file.

This is mainly name address phone, & etc. I would like to use a 5 X 8 card
stock.

My problem is I do not know how to set up word document to lock the fields
(if possible) so I just import the contacts data I wish to import. Can anyone
help?
