contact custom form icon is envelope

I've created a custom form which I have been using for a while. Now the icon
is an envelope.

The Reading Pane also says: This item contains active content that cannot be
displayed in the Reading Pane. Open the item to read....

I'm not sure if the two are related or not.

After searching here, I unchecked the block on properties to send
definition. I would gladly use the usual Outlook contact icon, but I can't
find it anywhere, therefore I just selected another icon.

Any ideas?? Thanks

Set the icon on the (Properties) page of the form. The default icons for Outlook items are stored in Program Files\Microsoft Office\OFFICE11\FORMS\1033 or the corresponding folder for your version of Office and regional settings.

Did That. In the manage forms screen the icon is correct. In any view,
e.g. phone list, it is still an envelopes.

Did you change both small and large icons to the correct file?

Yes I did. Also cleared the cache in both the folder properties AND the
Forms folder under outlook. Still have envelope!
