Confirmation Pg not working

I created a confirmation page for a form on my website
using the conf. page template in FrontPage. I linked the
conf. page to my form using the Form Properties dialog
box. When I submit the form, however, I get an error
message saying:
FrontPage Error
user: please report details to this site's webmaster.
Webmaster: see server's application event log for more
details.

What did I do wrong? Thanks

Can you post a URL??
Is the site running on a server with FrontPage extensions.?
Did you publish using http://websitename.com as the Publish to address??
