Combine multiple fill-ins

I have a document with 5 fill-in fields. In Word Perfect I could have all of
the fields come up in one prompt box. In Word each field comes up as a
seperate prompt. Is there a way in Word to have all five prompts come up in
one prompt box? TIA

You can do that by creating a "userform" or custom dialog box. See
