Clearing up Licensing entries in the License database

How do you clear out entries for old licenses (both
Exchange and Windows 2000) in the License database? I've
manually deleted them in the past but they keep coming
back. Computers that are no longer on-line keep
reappearing. We keep getting out of license errors and
it's been frustrating.
