Check Mark does not show in form check box

I have a form with two check boxes that the user may check their choice.

Upon receipt of the form the check marks do not show.

Any ideas?

Did you bind each check box to an Outlook yes/no property, on the Value page
of the control's Properties dialog? If you don't do that, Outlook does not
know where to store the data.

FYI, there is a newsgroup specifically for Outlook forms issues "down the
