word 2003 online form automatic check box

we are trying to put check boxes into an online form that will allow users to
fill out form and make a choice by just checking the box. The form has been
previously created and the check boxes must be inserted into the form. This
is not a check box that you just put an "X" in, it is an automatic check box
that shows a check mark when clicked. HELP?!?!
