change format of phone field from text to phone

Just recently when I enter a phone number it appears as 1112223333 not
(111) 222-3333 like it should. When I go into change the format the only
options is the test field. How do I change this back to how it was.

That behavior usually means your default dialing location is not set
correctly for your operating system. See if it is.
