I have been working on a very similar problem with Windows
XP and have done everything you listed including removing
the IDE cables and re-installing each drive one at a
time. I can see my CD-DVD, CD-R/W, and my ZIP drive
listed in explorer and in "My Computer" but can not read
media on any of the 3 drives. All three drives are
reported to be "HEALTHY" and working. Using command each
drive reports "The parameter is incorrect" My hardware
vendors are not aware of this problem.

HOWEVER, I left a CD in the drive when I powered off my PC
and discovered a "SURPRISE" the drive with the media
present during power up works and the drives without media
at power up do not work. Therefore, I booted my PC with
media in all three drives and all three drives work.
THIS IS VERY NEW, it never operated this way before,
having media in a drive at power-up was NOT needed.

SO try it, but now we need to determine cause for the
change. BTW I re-installed the Upper/Lower filters
after I discovered this work-around and that had no effect
either way, sounds like the filters were not an issue.
HELP!!! New Ideas needed.
