Can't print - IE6, Windows Server 20003

I am unable to print at all from IE. I have even tried to print from
about:blank and get the same error:

An Error Has Occurred In The Script Of This Page:

Char: 1
Errpr: dialogArguments.__IE_PrintType' is null or not an object
Code: 0
URL: res://c:\windows\system32\shdoclc.dll/preview.dlg

If I browse to this URL it does the same thing. My question is why is
it trying to pull up this dialog at all? It appears when I click the
print button, go to File, Print, or File, Print Preview.

I have followed the advice found so far with disabling 3rd party
extensions (was disabled), verified sunfairy is not installed.

I am running Windows 2003 Server, Web Edition.
I have all current patches installed, including
828750-MS03-040: October, 2003, Cumulative Patch for Internet Explorer

I have reregistered DLLs as per kbase including
URLMON.DLL, actxprxy.dll, iepeers.dll, Shdocvw.dll, jscript.dll,
Mshtml.dll, Browseui.dll, msxml.dll, scrrun.dll

Here are version numbers of some of the DLLs
SHDOCLC.DLL v6.0.3790.0
shdocvw.dll v6.0.3790.85
dispex.dll v5.6.0.8515

IE About reports: Version 6.0.379.0, Update Versions:0

1. If Q828750/Oct-03 Cumulative Patch for IE isn't installed, install it via
Windows Update (AV app disabled, please).

2a. Preview.dlg errors
