Can't Get Filename in Footer

Recently, I have been unable to get my document filename to appear in the
footer section of the page. Instead I get the following:

 FILENAME \p 

I have also been getting errors which say that Word has made corrections to
errors in the document and a dialog box called "Show Repairs" and the repair
is:

"Internal Data Integrity (Type 4) 1"

I have taken my Office disk (2003) and clicked on the "repair" button.

This did not solve my problem.

How do I rectify this?
