cant get a video card to run

Im trying to get this video card going, i set it up in my
computer and i hooked up the monitor to it but when i do
the monitor turns on but black and grey squares appear
and they wont go away so i cant go and do the things i
want, what should i do?

Is an AGP card? is it the correct voltage for the motherboard? is it
properly shoved into the slot? are the cables properly connected?

Does it not display anything or just when it gets to windows?
