Cannot delete custom form

Microsoft Outlook custom form question

I've created a custom form to use when sending messages. I accidentally
saved a copy of the form in a personal outlook folder instead of the
intended Organization Forms. When I tried to delete this form I found that
the "Delete" button was grayed out.

Using Tools->Forms->Choose Form menu option I was able to see and select
this form. but I cannot delete it! It turns out that the "Delete" button is
always grayed out no matter which folder or which forms I am dealing with.

I am a domain admin, an exchange admin and a local admin on the Outlook
machine. so I'm pretty sure it isn't permissions causing my problem. Does
anyone have any ideas on how to rid myself of this erroneous form?

Choose Tools | Options | Other | Advanced Options | Custom Forms, then click
Manage Forms. That's where you'll find a Delete button. You can also get
there through the Properties dialog for the folder where you published the
form.
