Note that you never asked HOW. You merely asked IF. And the answer was
stated that yes, it can be done. Note also that many of us answer thousands
of questions here in the newsgroups -- for free.
Some questions, however, like yours, could take hours/days/weeks to address.
You didn't give nearly enough detail to allow us to even attempt a useful
answer. I, for one, am not even sure what questions to ask.
So, let's start:

First, I don't know what a "change request form" is. There is no context.
There's nothing in Word called "change request form," so I'm left to guess
what you're talking about. Or, is this relevant to your question?
Second, you said "form". Do you mean a Word form, as in "protected for
forms" or are you using form in a generic sense? The answer to this makes a
great deal of difference in how the question should be answered and the
approach to suggest. Some things don't work in Word forms, or don't work
easily.
The "jpg will show an example of the change". I don't know exactly what that
means. My first guess is that the content of the jpg is relevant to the
question. So, are you saying that when the user clicks a button, Word will
then generate a jpg to show the effect of a given change? Or will there be
an existing library of jpg files that will be called upon depending on
exactly what type of change you're talking about?
If it's the latter, how would the underlying VBA be able to tell which jpg
to display? Or you going to have change-type checkboxes that make this
determination?
The questions just go on and on before one could begin to suggest an
approach.
That's why the first responder said "Yes" to "Can this be done?"
Yes, it can be done... I think. But, not easily. It sounds like it will take
a lot of work. I don't mean a 15 minute answer. I mean days, and possibly
weeks, depending on how involved it is.
While many of us answer a lot of questions for free -- some of them rather
involved -- there are some questions that seem to go well beyond the kind of
help one can reasonably expect in a peer-help forum. It's rather like going
onto a Health forum and asking one of the doctors for precise step-by-step
instructions on how to perform open heart surgery.
I'm sorry if we came off as rude or abrupt. But, to some of us, the question
really came off like one of those "Can somebody do my job for me?"
questions, and I don't believe that the unpaid volunteers here (some MVPs
and some not) are willing to go quite that far. On the other hand, most of
us are willing to meet you at least halfway. If you can articulate your
question in a way that tells us exactly what you're doing, I'm sure that
someone here will try to pitch in and help.
