Depends on what you want to create. Access can send EMail; it can keep
a list of people with their birthdates, and you can easily calculate
this year's birthday anniversary with an expression

DateSerial(Year(Date()), Month([DateOfBirth]), Day([DateOfBirth])

However, it's not designed as a graphic design program, so if you want
cute cartoons, colorful balloons and the like, you'll need some other
program to generate them.
