attachments not showing in sent items

I have one user being affected by this and have no clue

w2003 terminal server
exchange 2003
office 2003

when we send attachments and check the sent items we get

[Error : unloaded OLE object or mail attachment

and the attachment is not there. the email does get delivered with the
attachment. no other users are affected by this.
