Attachment question

I have a general question that is driving me crazy.

When you attach a document to an outlook email how come some times it puts
the icon of the document in the body of the email and sometimes it creates an
attachment line under the subject line? I notice that when I copy/paste a
document it is 50/50 chance of what is does. What is the criteria of doing
one over the other?
I am running XP with Office 2003

It depends on the message format. Only rich-text messages show attachments
inline.
