Alt-Tab (or clicking a URL) results in add'l windows coming forward

I've searched Google and this NG and haven't seen my exact situation
described or resolved so I'll try asking for myself.

When I Alt-Tab to switch back to the last-used application, some other
application that I haven't used in a while will come forward, too (but not
always). Sometimes that 2nd application is one that was minimized. It's
like Alt-Tab is unclear that I just want the last application, or that the
ordering of applications in the list gets fubar'd.

I've also seen this behavior when I click a URL in, say, an Outlook email
message. I keep one IE7 window open for various tasks but rarely switch to
it so when I click a URL in an Outlook message, I get not only the new IE7
window but also the old IE7 window comes forward behind it. Then, if I want
to Alt-Tab, Outlook is not the application that I would be switched back to.
It's the old IE7 window. I have to hit Alt-Tab-Tab to get back to Outlook.

I'm all SP'd and patched and whatnot and running Windows XP Pro.

Surely I'm not the only person to whom this has occurred.
