After Windows update, new file: epswad4.exe???

After a Windows update all of a sudden i have a new file that pops up on the
desktop: epswad4.exe????

Not only that, but now some of my desktop shortcuts no longer work unless I
open "run as" then close it.

Every time I get a windows update it takes me days to figure out why
something doesn't work anymore. I've turned the damn Windows Update OFF!
