Adding "Create date" to OL2003 contact form

I would like to add a couple of new fields to my OL2003 contact form: one
would automatically note when the particular contact was created, and one
would note the date when it was last edited. I am no VB Script expert...is
there a simple way to edit the standard form to add these two fields, or am
I whistling in the wind?

Also, i recently downloaded the Personal Folders Backup add-in from
Microsoft and installed it successful. However, I cannot figure out how to
use it. The instructions tell me to go to File --> Backup but there is no
"backup" option. I ran a search for the program, pfbackup.exe, but it's
nowhere on my hard-drive. Very mysterious!

Those fields are already available and visible on the All Fields tab as the Created and Modified dates under the All Contact Fields list. If you want to add them to a custom form page, just drag them from the field chooser to that page. See http://www.outlookcode.com/d/newdefaultform.htm for information on making your custom form the default.
