Adding a Title and Picture to appear on all slides

I know this has to be a easy thing to do but I have been away from PowerPoint
for quite some time.

I am trying to a presentation with a name and logo to appear on all slides.
I tried adding it to the master and it didn't appear on all of them, and I
played with the title slide but couldn't locate how to add it to all the
slides.

I also looked under View - Header and Footer but there is only one field.

What am I doing wrong?

Joey,
You are right you should add your text and logo to PPT's masterslide.
If you are using PPT 2003 and add the logo only to the master slide it will
appear on all slides except those formatted with the title slide design.
If you want your logo there also you must add a title slide. To do so go to
the slide master and on the slide master toolbar click the second button
from the left 'insert title slide'. Normally the logo will also appear on
that title slide.
If you are using PPT2007 on the other hand, you will have to add the logo to
the mother master slide, the one on the top, slightly to the left. If you
add it there all the slide designs will inherit it. So will the title design
which you do not have to add anymore.

Correction my first sentence should have read:
You are right you should add your text and logo to PPT's master slide.

My second should have read:
If you want your title slide to be formatted differently you must add a
title slide.
