Adding a header and footer

I have a form that I want to add a header to. Is this possible? I can see the header in design view but it disappears in form view

The Form Header displays in Form View but not when the form is
printed.
The Page Header prints out, but does not display.

Why would you want to print a form?
Make a report. That's what reports are for.
