missing header/footer

All of a sudden my form's header & footer is gone in Form View. They are
present in Design View.

I've tried turning off the View-Page Header/Footer on the menu & turning it
back on. (I have to repaste the stuff when I do this.) But this doesn't help.

You need to use the Form Header/Footer to display in form view, not
the Page Header/Footer.
Page Headers and Footers only display when the form is Printed.

Also, the Form Header and Footer have a 'Display When' property. Make
sure the correct option (Always) is selected.
