Pivot Table Form - Headers and Footers; Access 2007

I've successfully created a pivot table form using a select query. Now I want
to add a Title, page numbers and form footer. On the design page Access
allows this, but it will not display in View, print preview, and print. This
format with headers and footers will work great for my customer because they
can filter further, as appropriate.
