active window not coming to foreground

I am having a problem that occurs intermittently, but several times a
day. If I click on a running program's icon on the taskbar, the program
will become active (its title bar turns blue, as opposed to grey), but
it doesn't come to the foreground. Sometimes, I can resolve this by
re-clicking the task bar icon twice. Other times, I need to minimize
all of the windows in front of the active window.

This is not a mouse hover issue, as I have that feature disabled.

Any ideas?
