Access and Emailing an Approval Form

I am hoping access can send an email, in the email I want to embed a form with a YES/NO selection box. If the box is checked yes (approved), and replied to, that Access can see that the selection box is TRUE and email the Buyer to initiate the Purchase.

Is this possible, and if so, can someone please give me some insight on how to implement this?
