Access a Ftp Site using my own form in Frontpage 2003

Hi, I want to know if is possible to obtain a example form to make my own
form to access my FTP Site?.

I work in Frontpage 2003 and I have a Windows Server 2003 Web Edition with
the Frontpage 2002 Extensions running.

Use the feedback form as an example form layout (File->New and choose page
templates and feedback page from the Task pane)

For a form handler, search for an appropriate one in whichever technology
(ASP, ASP.NET, PHP, Perl/CGI etc.) you wish to use.
