2000 Send fax fails after Finish

I've found this in the archives,

But no steps provided relief.

"There was an error found when printing the document <document name> to
MSFAX: do you want to retry or cancel?"

I can print to file when the fax is selected.
I can set the fax to OFFLINE and it shows 2 in the queue (although I see
nothing when I open the printer queue detail) - when I deselect OFFLINE, it
gives me the error immediately.

No errors in event log - fax console is set to Maximum logging - only shows
service starting & stopping.

All other printing functions are working normally.

OK John, I am committed to solve this issue but only with your co-operation.
I talked to the printing team & got some diagnostic steps for you. Please do
the following & let us know what happens.

MSFAX: is the name of the port for Fax Port Monitor. We believe the Fax Port
Monitor is unable to create temporary files or is not able to write to the
file. Install FileMon from http://www.sysinternals.com/files/NTFILMON.ZIP.
Do following

(1) Open FileMon & put 'spoolsv.exe' in 'Include' text box on 'FileMon
Filter' Dialog
(2) Repro the problem - basically try to send a fax. You'll get the failure
(3) Save the output in FileMon & send us the log.
