Task Updates No Longer Working

Task Updates no longer work when the 2nd person on the task chain then
assigns it to a 3rd person. The update chain is broken.

Person A assigns to Person B with both "Keep an Updated..." and "Send me a
status..." boxes checked. Person B can accept and update normally. However,
if Person B assigns to Person C, Person A no longer receives updates or
status reports and the update chain is broken.

We use tasks extensively and this is seriously hurting our business. Any
help would be appreciated.

version of Outlook on each PC
platform used

What happened between when it did work and now when it's not working.

What happens now?
