Outlook 03 starts in offline and prompts for username & Password

I have a user who is using Outlook 03 in cached exchange mode, and is
connected to a 2003 Exchange server.
Everytime He opens outlook it is in the "offline" state. When he unchecks
Work Offline, it then prompts for username and password. User types username
& password, but it keeps prompting. It recently just started doing this.

Things I have tried.
*User logs into OWA fine with same username and pasword just fine.
*Unchecked Work Offline, closed outlook and restart and it still starts in
"Work Offline"
*User has access to all the network drives, intranet, etc... that he should
have access to, but username and password do not work when prompted in
Outlook.
*I have verifed that "automatically detect connection state" is selected.
*I have verified that "Always prompt for username and password is not checked.

Any suggestions.

Figured out the problem. The problem was the user had 3 different entries in
the Stored Usernames & Passwords. I cleared all the entries and the user is
not prompted anymore for Credentials.
IN XP...Control Panel-->User Accounts-->Advanced-->Stored Username and
passwords.
