Index.htm not being updated

On my laptop I build my site and then publish any updates
to the server. The index file on my laptop is
Index.htm. Now recently when I post updates the original
index.htm stays on the server and the new/updated
Index.htm gets published as Index.html

Does anyone have any ideas why the Index.htm is not being
properly updated?

My publishing settings are:
Publish - Changed pages Only
Changes - Determine changes by comparing source and
destination webs.

I have tried using:
Publish - All pages, overwriting pages already on
destination
Changes - Use source file timestamps to determin changes
since last publish

but I have the same result.

Thanks for any input.

You need to contact your host and ask if they can make index.htm is the first default document name
in the list of default documents for your web site.

Because index.html is what is required by the server for the default page.
Rename the local index.htm to index.html
Publish.
Then open the live site and delete index.htm if it exists.

If you publishing to a Unix server, index.htm and Index.htm are two
different files.
On your local web, rename Index.htm to index.html, allowing FrontPage to
update all hyperlinks. Then rename index.html to index.htm, again allow FP
to update hyperlinks.
The two stage rename process is required since Windows treats Index.htm and
index.htm as the same names.
If your host accepts index.html as the default page, omit the second rename
step.
If you use it, check navigation view for any problems - you may wish to make
a backup of the site beforehand, just in case.

Thanks for the suggestions. I have renamed the index.htm
on the Apche web server to index.html. I then had to
manually update the hit counter (I know, thy're lame)
because it defaulted back to 1 hit.

I have renamed the index.htm file locally also and will
see what happens when I republish tonight
