Document attachments

I'm using Outlook 2003.

When I attached documents (Word, Excel, PowerPoint) to an Outlook email,
sometimes the attachment icon is on the body of the email itself. Sometimes
it ends up on its own line underneath the Subject line.

Two questions: What determines where the icon is attached in the email?
Also, I work with a faculty member who prefers to receive an email attachment
when it is on a line under the Subject line -- is there a way I can get an
attachment to do this?
