Custom Contact Form Displaying Correct Email Address For Email 1,2,3

I created a custom contact form. I copied out the email selection and
email display from the O2003 default contact form. When I run the
custom form, changing the email does not change the email display. It
works fine with the O2003 default contact form. Is this a limitation
to creating a custom form?

More a limitation of those controls that are basically private to the
Outlook application. It also turns out that email and telephone
formatting are the first things to be lost if you try to customize the
General (first) tab of the contact form. So, it is no surprise that
the special formatting ability is lost when you copy the controls to a
custom form. Remember, you aren't copying the code that lies behind
the form that does the special formatting.
