Add to Quick Launch Bar Crashes Explorer

New notebook running Vista 32bit Home Premium... every time I try to use
"Add to Quick Launch" from the start menu explorer crashes... the error says
the fix is the reliabilty update, but that's already installed and if I
download and try to install it again, it says it's not needed...

PLEASE HELP !!!
